- • Protects and maintains the security and confidentiality of patient data and medical information in accordance with all applicable policies and procedures.
- • Ensures integrity of the Master Patient Index and all clinical documentation images by performing prepping, scanning, indexing, and quality audits of medical record documents with high accuracy and timeliness.
- • Facilitates electronic health record documentation completion by identifying deficiencies and assisting physicians in the timely completion of electronic records.
- • Handles all aspects of preparation and distribution of information from the patient’s legal health record as requested by authorized individuals and hospital departments.
- • Assesses and verifies scanned images against quality and accuracy standards, monitors manual index queues of unassigned images, and validates demographic information including patient merges and baby name changes.
- • Validates interfaces to ensure accurate receipt of clinical information from various systems into the electronic legal health record.
- • Performs analysis of clinical documentation, assigns physician deficiencies, and tracks completion to meet regulatory agency requirements.
- • Captures paper medical records into Horizon Patient Folder, manages electronic images within HPF, and maintains scanning equipment according to established procedures.
- • Assists with tracking of unbilled accounts by troubleshooting Discharged Not Final Billed reports, resolving registration data errors post-discharge, and utilizing inter-departmental shared files to support Accounts Receivable goals.
- • Reconciles paper medical records with hospital census to confirm receipt of all patient records.
- • Ensures clinical documentation flows appropriately to internal and external audit work queues and participates in quality audits and reviews.
- • Maintains secure and accessible paper record storage areas until original documents are eligible for destruction.
- • Discloses patient information only to authorized parties and ensures all disclosures comply with applicable privacy and regulatory guidelines.
- • Demonstrates strong knowledge of medical record format and content for both inpatient and outpatient visits.
- • Performs all duties in a Microsoft Windows environment with proficiency in required software systems.
- • Operates scanning hardware and maintains equipment in accordance with departmental procedures.
- • Makes sound decisions in high-volume, detail-oriented tasks while maintaining accuracy and compliance.
- • Communicates effectively and collaborates with physicians, clinical staff, and cross-functional teams to ensure seamless health record management.
- • Adheres to all HIM policies, HIPAA regulations, and organizational standards for patient data handling.
🎯 Requirements
- • High School diploma or equivalent
- • Strong knowledge of medical record format and content for inpatient and outpatient visits
- • Ability to perform computer functions in a Microsoft Windows environment
- • Ability to be detail-oriented and perform tasks at a high level of accuracy
- • Previous experience in a hospital HIM department preferred
- • Previous experience with an electronic legal health record system preferred
