• • The Hospice Home Care Nurse provides essential nursing and supportive care to terminally ill patients and their families in home and facility settings, ensuring comfort, dignity, and quality of life during end-of-life care.
• • Day-to-day responsibilities include conducting initial patient and family assessments, developing and implementing individualized care plans, administering medications, managing IVs and wound care, coordinating with interdisciplinary teams, and educating families and facility staff on care techniques.
