- • Deliver compassionate, high-acuity bedside care in the comfort of patients’ homes throughout Sacramento County. You will be the clinical eyes and ears in the field, performing hands-on interventions such as PICC-line maintenance, wound-VAC therapy, Kangaroo feeding-pump management, NG/G/J-tube care, staple/suture removal, drain management (JP, biliary, nephrostomy), Foley and suprapubic catheter insertion/removal/troubleshooting, PleurX drainage, tracheostomy care, blood draws, ostomy management, SQ/IM injections, IV antibiotics, TPN administration, and compression wrapping.
- • Execute the plan of care developed by the Case-Management RN, ensuring every intervention aligns with the patient’s goals, physician orders, and California’s Nurse Practice Act. Your assessments drive real-time clinical decisions and directly influence readmission rates that already sit 5+ points below the national average.
- • Document accurately and completely within 24–48 hours using our point-of-care system. Your notes paint a concise picture of the patient’s status, response to treatment, caregiver capability, and emerging needs, enabling seamless communication with the interdisciplinary team (PT, OT, ST, MSW, HHA).
- • Educate patients and family caregivers in plain language they can absorb and apply. Whether teaching sterile technique for a new trach or demonstrating how to troubleshoot an alarming feeding pump, your teaching reduces anxiety, prevents complications, and fosters independence.
- • Collaborate in weekly case conferences, presenting your findings and flagging high-risk changes before they escalate. Your voice influences staffing decisions, supply orders, and even agency-wide clinical protocols.
- • Maintain productivity and visit efficiency without ever sacrificing quality. You will manage a daily route that balances geography, acuity, and urgency while keeping mileage and drive time within reasonable limits.
- • Participate in rotating weekend on-call coverage, providing triage and urgent visits that keep patients safely at home and out of the ED. You’ll have 24/7 phone access to a supervisor and an on-call physician for clinical support.
- • Uphold HIPAA and agency privacy standards at every doorstep. Your professionalism and cultural sensitivity build trust in diverse neighborhoods across Sacramento’s urban core, suburbs, and rural outskirts.
- • Embrace continuous learning through monthly skills labs, annual competency fairs, and tuition assistance for specialty certifications such as OASIS, wound care, or infusion therapy.

🎯 Requirements
- • Current, unrestricted Registered Nurse license in the state of California
- • Minimum one year of recent acute-care or home-health experience with high-acuity patients
- • Reliable transportation and valid California driver’s license with proof of insurance
- • Proficiency with electronic medical records and point-of-care documentation systems
- • BLS certification (American Heart Association) required; ACLS preferred
- • Ability to lift and transfer up to 50 lbs and navigate stairs, uneven terrain, and varied home environments
