- • The Home Health Nursing Staff Scheduler plays a vital role in ensuring seamless coordination of patient care by managing scheduling operations for home health services, directly impacting patient outcomes and organizational efficiency.
- • Day-to-day responsibilities include coordinating patient appointments, managing clinical documentation, verifying time and travel entries, monitoring compliance with supervision visits, and providing telephone and in-person customer service support to patients, families, and healthcare providers.
- • The role is based at the Stonerise Home Health Services South Charleston Agency, part of CommunityCare Health Services, Inc., a mission-driven organization committed to enriching lives through compassionate, principled, and purposeful care.
- • In this role, the individual will develop expertise in healthcare operations, medical scheduling systems, and patient coordination while contributing to a supportive, team-oriented environment that values initiative, professionalism, and service excellence.
🎯 Requirements
- • High school diploma or equivalent; associate degree or related education preferred
- • Current LPN license preferred
- • Proficiency in Microsoft Office applications
- • Strong interpersonal and communication skills for interacting with patients, families, nurses, and healthcare providers
- • Detail-oriented with ability to manage multiple tasks, prioritize effectively, and maintain accurate records
- • Familiarity with medical terminology and comfort working with electronic health records and scheduling systems
