- • The Home Health Referral Coordinator II (DME) supports physicians and the interdisciplinary team in facilitating patient transitions from hospital to home with home health services, aiming to enhance clinical outcomes, patient satisfaction, and cost-effective care.
- • Day-to-day responsibilities include collaborating with physicians, nursing staff, and home health companies; identifying and resolving discharge delays related to durable medical equipment; collecting patient co-pays; validating home health orders; coordinating DME delivery and patient education; monitoring patient progress; ensuring care plan documentation; conducting patient home health screenings; adhering to safety and quality standards; and mentoring less experienced staff.
