You will be instrumental in delivering comfort, dignity, and support to individuals and their families navigating the end-of-life journey, embodying the profound principles of hospice care.
  • • As a Home Hospice Aide, your primary responsibility is to execute specific aspects of patient care as outlined in the individualized Hospice Aide Plan of Care. This plan, developed by a Registered Nurse, will dictate the frequency of your visits and the precise nature of the care required. Your day-to-day activities will encompass a wide range of personal care needs, ensuring prompt attention to patient and family requests. This includes assisting with essential hygiene tasks such as baths, back rubs, oral care, and shampoos, as well as changing bed linens to maintain comfort and cleanliness.
  • • You will play a crucial role in assisting patients with dressing and undressing, and providing support with feeding, always with a focus on the patient's comfort and autonomy. Safety is paramount; you will be responsible for identifying and meeting the patient's safety needs, ensuring all equipment, such as foot stools, side rails, and oxygen apparatus, is used safely and correctly. Your attentiveness will extend to monitoring and documenting vital signs, including oral, rectal, and auxiliary temperatures, pulse, respiration, and blood pressure, within the scope of your training and as ordered by the supervising nurse.
  • • A significant aspect of your role involves maintaining skin integrity. You will provide meticulous care and observation of the patient's skin, taking proactive measures to prevent the breakdown of tissue, particularly over bony prominences. Your keen observation skills are vital; you will be expected to report any changes in the patient's condition, no matter how small, to the assigned supervisory nurse. This includes being aware of the dynamics within the patient's household, such as other caregivers or family members, and any interpersonal issues that may arise, providing valuable insights to the nursing team.
  • • Beyond direct patient care, you will contribute to the patient's overall well-being by assisting with meal preparation, which may include grocery shopping as assigned. You will also support therapeutic goals by assisting with ambulation and exercises as instructed by the primary care nurse or therapist, and performing range of motion exercises and other simple procedures as an extension of therapy services, all within your defined training limitations.
  • • Light laundry duties and maintaining the patient's living area in a clean and orderly fashion, as assigned, are also part of your responsibilities, contributing to a comfortable and safe home environment. Furthermore, you will provide valuable respite for patients, families, and caregivers when you are on-site, offering a brief period of relief and support. This role demands a deep understanding and embodiment of the hospice philosophy, emphasizing comfort, dignity, and quality of life for patients and their loved ones during a critical time.

You must possess excellent oral and written communication skills to effectively interact with patients, families, and healthcare professionals. Familiarity with hand-held communication devices and electronic documentation systems will streamline reporting and communication.
