- • Provides intermediate direct patient care in home and alternate infusion suite settings under a physician’s plan of care, ensuring patient safety, continuity, and regulatory compliance.
- • Initiates, develops, and implements individualized nursing care plans, evaluating patient progress toward goals and modifying treatment based on patient status changes or physician orders.
- • Conducts comprehensive patient assessments during each skilled visit using health data access and patient interviews, re-evaluating needs through physical reassessment and response to therapy.
- • Administers medications, performs infusion treatments, and educates patients and families/caregivers on therapy protocols to promote independence and adherence to care plans.
- • Communicates timely and effectively with Option Care’s clinical team, medical providers, patients, and families to ensure seamless continuity of care and prompt reporting of significant clinical changes.
- • Documents all clinical activities in real-time during visits, adhering to Option Care’s policies for legibility, approved abbreviations, and error correction to optimize billing compliance.
- • Coordinates discharge planning, prepares detailed discharge summaries with patient instructions, and reports care progress, needs, and goals when transferring patient care.
- • Trains and mentors Infusion Nurse I staff to ensure patient safety, clinical competency, and adherence to agency policies and procedures.
- • Maintains strict compliance with agency operations, Option Care policies, professional standards, and all applicable local, state, federal regulations, and accreditation requirements.
- • Utilizes computer systems and automation tools to validate treatment orders, communicate care provided, and follow assigned visit schedules efficiently.
- • Responds promptly to patient requests and initiates emergency procedures as necessary, ensuring immediate clinical intervention when required.
- • Upholds ethical and professional conduct, safeguarding patient confidentiality and proprietary information in full compliance with HIPAA regulations.
- • Demonstrates knowledge of Option Care’s services and effectively instructs patients on financial obligations related to care and service charges.
- • Completes mandatory training modules, in-services, and continuing education to maintain licensure and clinical competency in home and alternate site infusion care.
- • Participates in multidisciplinary team conferences and provides precepting, training, and mentoring for new nurses during orientation and onboarding.
- • Assumes on-call responsibilities as assigned by the nursing department to support 24/7 patient care needs.
- • May insert PICC/Midline catheters per physician order, troubleshoot and declot obstructed central lines, and educate patients on infection prevention and routine line care.
- • Adheres to all safety protocols including use of closed-toe shoes, facemasks, goggles, gowns, gloves, and functional communication devices during routine and emergency situations.
- • Travels up to 100% of the time to provide care in patients’ homes and alternate infusion sites across Hunterdon County and surrounding areas.
- • Exerts up to 50 pounds of force regularly, with a maximum of 75 pounds, and performs physical tasks including walking, standing, lifting, bending, reaching, and balancing.
- • Is exposed to blood, body fluids, infectious materials, hazardous substances, sharps, chemicals, extreme weather, and unpredictable home environments during daily duties.
