Full Job Description

đź“‹ Description

  • • Lead the charge in safeguarding the highest standards of hospice care as Mission Healthcare’s Hospice Quality Care Manager (RN). In this fully remote role you will serve as the clinical compass for a seven-state organization that touches thousands of lives each year, ensuring every patient receives dignified, evidence-based, and regulation-compliant care from admission through discharge.
  • • Own end-to-end clinical documentation review: scrutinize Plans of Care, physician face-to-face notes, skilled-need narratives, and HOPE assessments for accuracy, completeness, and medical necessity. Your eagle eye prevents costly denials and keeps patients on the right care trajectory.
  • • Translate complex Medicare Hospice Conditions of Participation and CHAP standards into practical guidance for field clinicians. When documentation gaps appear, you provide concise, respectful feedback that educates and elevates the entire care team.
  • • Drive performance improvement by mining SHP analytics in real time. Spot trends in HOPE accuracy, length of stay, or symptom management scores and partner with the Director of Quality & Education to design targeted education, policy updates, and workflow tweaks.
  • • Champion regulatory readiness. Whether it’s a surprise CMS survey or an internal audit, your meticulously maintained records and proactive risk mitigation ensure Mission Healthcare consistently exceeds expectations.
  • • Collaborate across time zones and disciplines. From multi-disciplinary case conferences to weekend on-call rotations, you communicate with clarity and empathy—bridging the gap between bedside nurses, medical directors, coders, and billing teams.
  • • Master the art of prioritization. Each morning you triage a queue of HOPE assessments, additional orders, and recertification packets, ensuring nothing stalls patient care or month-end close. Your ability to juggle competing deadlines keeps revenue flowing and families confident.
  • • Serve as the go-to resource for Homecare Homebase super-users. When clinicians struggle with workflow or documentation templates, you provide just-in-time coaching that reduces clicks and burnout.
  • • Model Mission’s CARES values—Compassion, Accountability, Respect, Excellence, Service—in every interaction. Your feedback is never punitive; it’s a teaching moment that reinforces our culture of caring.
  • • Contribute to strategic growth. Insights you gather from chart reviews and SHP dashboards inform executive decisions about new markets, service lines, and technology investments.
  • • Enjoy the autonomy of remote work while staying tightly connected. Daily huddles, instant messaging, and virtual coffee breaks replicate the camaraderie of a physical office without the commute.
  • • Work Tuesday–Saturday, 8 a.m.–5 p.m. PST, ensuring west-coast-centric coverage while preserving your Sundays and Mondays for personal pursuits.
  • • Receive robust support for multi-state licensure. Already licensed in California? Mission will sponsor and guide you through obtaining RN licenses in Oregon, Utah, Idaho, and Nevada—expanding your professional footprint at no cost to you.
  • • Thrive in a culture that values your voice. Monthly diversity-and-inclusion roundtables, employee resource groups, and open-door leadership mean your ideas shape the future of Mission Healthcare.
