Integrated Care Assistant I, National IKC- Remote

📋 Description

• • Confirm the status of existing hospital admissions and identify new admissions and discharges for Transition of Care (TOC) support.
• • Conduct bi-weekly check-in calls with skilled nursing facilities (SNFs) to verify admission status and collect updated patient information.
• • Request, obtain, and upload medical records including lab results to support care team workflows.
• • Process admission and discharge summaries upon confirmation of patient transitions.
• • Provide administrative support to care teams by addressing ad hoc TOCs and SNF admits as they arise.
• • Monitor incoming faxes, process outbound faxes, and maintain an accurate fax log for documentation and compliance.
• • Complete manual mailing requests as required for patient communications and documentation.
• • Perform provider verifications to ensure accurate and up-to-date contact and credentialing information.
• • Collect, verify, and enter patient and admission data into IKC databases to support the Integrated Kidney Care Model of Care.
• • Assist with other administrative and operational duties as assigned to ensure seamless care coordination.
• • Communicate clearly and professionally via phone, email, and fax with patients, providers, and internal care teams.
• • Maintain high attention to detail when handling sensitive patient information and administrative documentation.
• • Ensure all tasks are completed accurately and in a timely manner to support patient care continuity.
• • Collaborate with care teams to resolve discrepancies in patient records and admission statuses.
• • Adhere to all organizational policies and compliance standards regarding patient privacy and data handling.
