📋 Description
- • Serve as the primary contact and coordinator for services supporting individuals with mental health, substance use, and intellectual/developmental disabilities across residential, school, workplace, and community settings.
- • Coordinate and monitor all services and supports outlined in each individual’s Person-Centered Plan (PCP), ensuring alignment with service goals and regulatory requirements.
- • Supervise, direct, and evaluate the performance of paraprofessional and clinical staff, including hiring, training, assigning work, appraising performance, and implementing corrective actions.
- • Provide direct clinical supervision to all employees to ensure adherence to best practices, ethical standards, and service protocols.
- • Respond as a first responder on a 24/7/365 basis to consumer emergencies or crises, coordinating appropriate resources in alignment with the PCP and immediate needs.
- • Deliver individualized training to staff on the knowledge, skills, and abilities required to effectively serve the specific population and age group supported.
- • Complete all required service documentation, including daily full service notes with the person’s name, Medicaid ID, date, purpose of contact, interventions performed, time spent, effectiveness, and staff signature and credentials.
- • Submit and manage Service Authorization Requests to ensure all services are properly authorized and funded prior to delivery.
- • Minimize the impact of psychiatric symptoms or substance dependence on daily functioning and personal development through targeted interventions and support planning.
- • Maintain working knowledge of North Carolina’s human services delivery system and ensure compliance with state-specific regulations and service definitions.
- • Ensure all staff documentation meets Medicaid and regulatory standards for accuracy, timeliness, and completeness.
- • Collaborate with interdisciplinary teams to standardize procedures, improve communication, and accelerate implementation of Person-Centered Plans.
🎯 Requirements
- • Must meet Qualified Professional (QP) criteria as defined by North Carolina service regulations: either a licensed/certified human services professional with four years of MH/DD/SA experience, a Master’s degree in a human service field with one year of post-graduate MH/DD/SA experience, a Bachelor’s degree in a human service field with two years of post-bachelor’s MH/DD/SA experience, or a Bachelor’s degree in any field with four years of post-bachelor’s MH/DD/SA experience.
- • Valid driver’s license and auto insurance required.
- • Employee supervisory experience preferred.
- • Experience with Electronic Medical Records (EMR) systems preferred.
- • Must pass pre-employment criminal background check, OIG exclusions database check, Child Abuse Registry check, and Offenders Against Individuals with Developmental Disabilities registry check.
- • Must pass drug screening and provide education verification and professional credentialing as required.
