- • Conduct comprehensive in-home assessments for health plan members aged 65 and over, including obtaining detailed medical histories, performing physical exams, reviewing medications, and administering cognitive, depression, and safety screenings.
- • Deliver patient and family/caretaker education focused on close monitoring, follow-up care, and management of chronic conditions.
- • Identify and document diagnoses for active medical management, with a primary emphasis on chronic disease management in frail and complex geriatric populations.
- • Collect laboratory specimens in the home setting when clinically appropriate and within scope of practice.
- • Communicate assessment findings clearly and promptly to primary care providers (PCPs) to identify and address gaps in care.
- • Engage in end-of-life care discussions with patients and families, demonstrating sensitivity and clinical competence in palliative and hospice-related conversations.
- • Coordinate access to community resources and support services to enhance patient care outcomes and promote cost-effective, holistic care.
- • Adhere strictly to all HIPAA regulations and maintain the security and confidentiality of protected health information (PHI) during home and virtual visits.
- • Perform duties within an interdisciplinary team that includes physicians, nurse practitioners, physician assistants, case managers, and other healthcare professionals to ensure coordinated, patient-centered care.
- • Conduct both home visits and occasional outpatient clinical work, with each visit lasting between 45 and 60 minutes depending on whether it is an initial or annual assessment.
- • Maintain compliance with all state and federal healthcare regulations and Alignment Healthcare’s internal policies.
- • Utilize electronic medical records (EMR) systems to document assessments, care plans, and clinical findings accurately and in a timely manner.
- • Demonstrate strong problem-solving, organizational, and time management skills to operate effectively in a fast-paced, high-volume home-based care environment.
- • Maintain professional, empathetic, and effective communication with patients, families, caregivers, and clinical team members.
- • Possess and maintain a valid California driver’s license and current automobile insurance to safely perform home visits throughout Riverside County.
- • Ensure active licensure, certification, and credentials are maintained in accordance with state and company requirements.
