  • • Provides direct patient care under the supervision of a registered nurse or physician, adhering strictly to established plans of care and physician orders.
  • • Delivers safe, competent nursing care in accordance with licensed practical nurse scope of practice and regulatory standards.
  • • Administers medications and monitors patients for adverse reactions to treatments or pharmaceutical interventions.
  • • Maintains accurate, timely documentation that supports coordinated patient care, meets regulatory requirements, and ensures proper reimbursement.
  • • Reports daily patient observations, changes in condition, and clinical concerns to the supervising Registered Nurse (RN).
  • • Communicates effectively with physicians, family members, and interdisciplinary team members regarding patient progress and care needs.
  • • Performs point-of-care testing in compliance with facility policies and procedural guidelines.
  • • Maintains constant alertness and adherence to safety protocols while working in a hospice care environment.
  • • Conducts routine physical tasks including standing, walking, lifting, and moving patients up to 50 lbs, as well as frequent bending, stooping, and repetitive hand/arm movements.
  • • Uses vision and depth perception for both near and distant tasks, including reading monitors, charts, and identifying color changes in patient condition.
  • • Relies on hearing to detect patient sounds, alarms, and verbal communication from patients and staff.
  • • Uses smell to recognize odors indicative of patient health changes or environmental concerns.
  • • Engages in frequent oral communication to relay critical patient information to clinical team members.
  • • Performs keyboard/data entry tasks as required for documentation and reporting.
  • • Works in a PRN/per diem capacity with flexible scheduling across shifts at the WI-Monroe Clinic Hospice Home.
  • • Operates within the legal and ethical boundaries of the Licensed Practical Nurse (LPN) license and facility policies.
  • • Completes all assigned duties with professionalism, compassion, and adherence to hospice care principles.

🎯 Requirements

  • • Graduate of an accredited school of nursing or educational equivalency for licensing
  • • One year of licensed practical nurse experience
  • • Valid Licensed Practical Nurse (LPN) license issued by Illinois Department of Financial and Professional Regulation (IDFPR) or compact state licensure (for Missouri, Oklahoma, Wisconsin)
  • • Current Basic Life Support HealthCare Provider (BLS HCP) certification from the American Heart Association (AHA)
  • • Ability to perform physical duties including frequent standing, walking, lifting up to 50 lbs, and patient handling
  • • Proficiency in oral communication, documentation, and point-of-care testing
