- • Devoted Health is seeking a dedicated and ambitious Manager, Advanced Practice Provider (APP) to lead and enhance our Transitions of Care (TOC) Program. This is a full-time, fully remote position requiring 40 hours per week, with specific working hours from Monday to Friday, 8:30 am to 5:00 pm MST or PST. As an APP Manager, you will play a pivotal role in shaping the clinical excellence of our growing TOC team, balancing direct patient care with significant leadership responsibilities.
- • In this dynamic role, you will be responsible for the clinical and operational oversight of the Advanced Practice Providers within the TOC program. You will be an integral part of the TOC management team, collaborating with the Medical Director, Operations Leads, Nurse Director & Managers, and Staff Supervisors. Together, you will foster a strong collaborative culture, empower frontline staff to achieve peak clinical performance, and proactively identify opportunities to elevate patient care standards.
- • A significant portion of your time, approximately 40%, will be dedicated to providing direct patient care within the TOC program. This involves managing patients during the critical 30-day post-discharge period. Your clinical responsibilities will include identifying and addressing gaps in care, escalating symptomatic patients through appropriate clinical pathways, providing necessary interventions, and ultimately working to reduce preventable hospital readmissions. The exact allocation of time to clinical duties may fluctuate based on the number of direct reports you manage.
- • The remaining 60% of your time will be focused on your managerial and leadership duties as an APP Manager and a key member of the TOC leadership team. This includes ensuring the clinical performance of the APP team meets and exceeds program standards. You will be accountable for the performance management of front-line TOC APPs, ensuring they meet required targets and deliver high-quality care.
- • You will be instrumental in developing, tracking, and managing key clinical and operational Key Performance Indicators (KPIs) specifically for the TOC APPs. This data-driven approach will help identify areas for improvement, allowing you to implement effective strategies to achieve targeted metrics and enhance program efficiency.
- • A core aspect of your role will be to manage, empower, inspire, and align the ever-growing team of TOC APPs. This involves conducting regular bi-weekly one-on-one meetings with each direct report to provide support, guidance, and feedback. You will also be responsible for setting personalized goals for your direct reports and conducting thorough reviews of their individual performance, fostering professional growth and development.
- • You will collaborate closely with the program's Medical Director and fellow APP Managers to refine team processes and workflows. This collaboration extends to enhancing interdisciplinary workflows within the APP team and across all roles within the TOC program, ensuring seamless patient transitions.
- • You will actively participate in the recruitment strategy, hiring, and onboarding of new TOC APPs, helping to build and expand a high-performing clinical team. Developing and refining clinical processes and policies for the program will be crucial to maintaining high standards of care and operational efficiency.
- • Monitoring clinical resource allocation and executing necessary changes to meet the practice's visit demands will be a key responsibility. This includes overseeing scheduling, managing time-off requests, monitoring daily, weekly, and monthly schedule density, and handling all schedule changes that impact patient appointments.
- • You will provide essential onboarding guidance for new providers, working in conjunction with the education and training team to ensure a smooth and effective integration into the program. Serving as the primary liaison to TOC leadership, you will provide updates on the status, challenges, and opportunities within the APP organization.
- • This role is ideal for a servant leader who understands that their success is tied to the success of their team. Experience with telehealth platforms, managing post-transition patients, and identifying the need for immediate interventions is highly desirable, as is experience managing acute/chronic disease exacerbations such as CHF, diabetes, COPD, and hypertensive emergencies. A proactive approach to process improvement and a willingness to embrace change using PDSA principles will be highly valued.
