- • Review clinical documentation and diagnostic results to accurately extract data and assign appropriate ICD-10-CM and ICD-10-PCS diagnosis and procedure codes for inpatient hospital records.
- • Ensure all coded services comply with ICD-10-CM Official Guidelines for Coding and Reporting, CMS regulations, and other mandated standard code set requirements.
- • Code a variety of inpatient cases including Medicare and Non-Medicare, surgical, medical, cardiac, maternal/neonatal, and trauma cases with high accuracy.
- • Maintain a minimum coding accuracy rate of 95% while meeting productivity standards of 2 inpatient charts per hour, subject to client-specific adjustments communicated by management.
- • Work daily on pending coding queues and GuideAudit review queues, ensuring all charts are processed and corrections are promptly communicated to the facility.
- • Query physicians when medical records contain ambiguous, conflicting, or incomplete information regarding reportable conditions or procedures, following facility-specific query policies and CDI reconciliation processes.
- • Correct and resubmit charts requiring re-bills in accordance with facility re-bill policies, ensuring timely communication to the facility for billing adjustments.
- • Adhere strictly to facility-specific policies, procedures, and coding protocols while maintaining full compliance with Guidehouse and client standards.
- • Notify administrative staff immediately if unable to meet the 24-hour turnaround requirement for coding assigned charts.
- • Respond to physician and hospital coding or billing inquiries within eight hours of receipt.
- • Clock in and out via ADP and accurately log billing hours in PeopleSoft using correct task codes.
- • Collaborate with client IT departments and Guidehouse IT to resolve technical issues impacting coding workflows.
- • Regularly review and utilize the Client Portal as a primary tool for coding abstraction and facility-specific guidance.
- • Check work email at least every two hours during scheduled work shifts to ensure timely communication and task management.
- • Maintain current professional credentials and complete all required continuing education courses provided by Guidehouse, submitting certificates in a timely manner.
- • Stay current with all Coding Clinic Guidelines and official coding updates.
- • Ensure HIPAA compliance by maintaining secure, protected workstations and safeguarding patient privacy in accordance with company handbook policy section 105.
- • Communicate professionally in all written correspondence, following the company email policy.
- • Work closely with IQC staff to meet and exceed quality assurance benchmarks for each facility.
- • Report downtime to administrative staff immediately to maintain client service level agreements.
- • Abide by all Guidehouse corporate policies and procedures as well as the policies of all client facilities.
