📋 Description
- • Conduct end-to-end triage of patient referrals and clinical documentation to determine urgency, risk, and appropriate care pathway using Montu’s policies and clinical best practices
- • Deliver structured telephone and video assessments to evaluate complex, multi-layered patient presentations, often after exhaustion of conventional care pathways
- • Identify exclusions, contraindications, and safeguarding concerns, escalating issues in alignment with Montu policy and national clinical guidance
- • Maintain accurate, timely, and compliant clinical records within digital systems, ensuring full auditability and data integrity
- • Signpost, refer, and escalate patients internally and externally to ensure seamless access to the most appropriate care resources
- • Champion clinical governance by actively participating in incident reporting, root cause analysis, and continuous quality improvement initiatives
- • Contribute to or lead audits of physiotherapy pathways, identifying gaps and recommending evidence-based enhancements
- • Develop, review, and update standard operating procedures (SOPs) for triage processes, ensuring alignment with regulatory and safety standards
- • Support the design and implementation of digital triage tools and patient education materials to improve efficiency and patient understanding
- • Participate in outcome measurement activities to evaluate the effectiveness of the triage service and inform service evolution
- • Play an active role in shaping the future of the physiotherapy function within Montu, contributing to service redesign and innovation
- • Work within a tech-enabled environment utilizing EMRs, telehealth platforms, and digital PROMs to deliver assessments and document outcomes
- • Adhere strictly to GDPR and data protection protocols while managing patient information securely and ethically
- • Work 40 hours per week, including one Saturday per month, with a day in lieu provided for Saturday working
- • Operate autonomously with strong clinical reasoning and decision-making skills, balancing clinical safety with service efficiency
- • Communicate complex clinical information clearly and compassionately to patients over phone and video consultations
- • Collaborate with multidisciplinary teams to ensure cohesive patient care pathways and integrated service delivery
- • Promote a culture of learning and improvement by sharing insights, mentoring peers, and contributing to team development
- • Engage in ongoing professional development to maintain currency in remote triage techniques, pain management, and digital health innovations
- • Represent Montu’s commitment to transforming lives through safe, effective, and accessible cannabis-based medicinal treatments
- • Adapt to a fast-paced, evolving healthcare environment where predictability is low and initiative, ownership, and innovation are rewarded
🎯 Requirements
- • HCPC-registered Physiotherapist with CSP membership or equivalent
- • Minimum 5 years post-registration experience, including at least 2 years at Band 6 or above
- • Demonstrable expertise in remote and telephone triage and safety-netting practices
- • Strong clinical reasoning and confident autonomous decision-making capabilities
- • Proficiency in using EMRs, telehealth platforms, and digital PROMs
- • Sound understanding of safeguarding, GDPR, and clinical governance requirements
