  • • Provide skilled occupational therapy services to patients in their homes across the Petaluma area, evaluating individual needs to promote safety, independence, and functional ability in daily living.
  • • Conduct comprehensive home assessments to identify environmental hazards and recommend modifications for home safety, including fall prevention strategies and adaptive equipment needs.
  • • Develop and implement individualized treatment plans focused on improving mobility, fine motor skills, coordination, and activities of daily living (ADLs) for patients with chronic conditions, dementia, or post-acute recovery needs.
  • • Educate patients and family caregivers on medication management, energy conservation techniques, pain management strategies, and proper use of assistive devices such as wheelchairs, walkers, and adaptive seating.
  • • Collaborate closely with Case Managers and the interdisciplinary care team to align therapy goals with overall patient care plans and ensure continuity of care.
  • • Document patient progress accurately and in compliance with regulatory standards, including Medicare, Medicaid, and private payer requirements.
  • • Advocate for patients by identifying unmet needs and coordinating with other healthcare providers to ensure holistic, patient-centered care.
  • • Maintain a caseload of patients across multiple homes in the assigned territory, traveling safely and efficiently to deliver timely, high-quality care.
  • • Participate in ongoing professional development and team training sessions to stay current with best practices in home health occupational therapy.
  • • Uphold 21HHS’s commitment to compassion and empathy by treating every patient with the same care and respect as one would show to a family member.
  • • Contribute to the agency’s low hospital readmission rates by delivering evidence-based interventions that reduce risk factors and improve long-term outcomes.
  • • Adhere to all agency policies, clinical protocols, and safety procedures while working independently in diverse home environments.
  • • Engage in regular communication with supervisors and team members to report patient progress, challenges, and resource needs.
  • • Support a culture of workplace excellence by participating in team recognition initiatives and contributing to the agency’s high employee satisfaction ratings.

🎯 Requirements

  • • Valid California Occupational Therapist license
  • • Current CPR certification
  • • Valid driver’s license and reliable transportation for in-home patient visits
  • • Experience in home health or skilled nursing settings preferred
  • • Ability to work independently and manage a caseload across multiple locations
  • • Strong documentation skills in compliance with Medicare and regulatory standards
