- • Provide skilled occupational therapy services to patients in their homes across the Petaluma area, evaluating individual needs to promote safety, independence, and functional ability in daily living.
- • Collaborate with Case Managers to develop, implement, and modify individualized treatment plans focused on home safety, medication management, wheelchair seating and positioning, pain management, falls prevention, chronic disease management, and dementia care.
- • Educate patients and family caregivers on therapeutic techniques, adaptive equipment use, and strategies to manage conditions such as dementia and mobility impairments within the home environment.
- • Conduct comprehensive home assessments to identify environmental hazards and recommend modifications that reduce fall risk and enhance patient accessibility and autonomy.
- • Serve as a patient advocate by coordinating with interdisciplinary care teams, including nurses, physical therapists, and social workers, to ensure holistic, patient-centered care.
- • Document all patient evaluations, progress notes, and treatment outcomes in compliance with clinical, regulatory, and insurance requirements.
- • Maintain consistent communication with referral sources and supervisors to update patient status, adjust care plans, and ensure continuity of care.
- • Adhere to agency protocols and clinical standards to maintain hospital readmission rates below 10%, exceeding industry benchmarks.
- • Travel daily to patient residences within the designated territory, ensuring timely and consistent in-home visits as scheduled.
- • Participate in ongoing professional development and training opportunities offered by 21HHS to enhance clinical skills and stay current with best practices in home health occupational therapy.
- • Contribute to a culture of empathy, compassion, and teamwork by supporting colleagues and upholding the agency’s mission of treating every patient as if they were family.
- • Recognize and respond to patient and family concerns with sensitivity, ensuring high levels of patient satisfaction and trust in care delivery.
- • Maintain accurate and timely documentation on all patient interactions, assessments, and interventions using agency-approved electronic systems.
- • Follow all OSHA, HIPAA, and state regulatory guidelines for home health care delivery and infection control practices during in-home visits.
- • Participate in team meetings, case conferences, and quality improvement initiatives to support organizational goals and clinical excellence.
🎯 Requirements
- • Valid California Occupational Therapist license
- • Current CPR certification
- • Valid driver’s license and reliable transportation for in-home patient visits
- • Minimum of one year of experience in home health occupational therapy
- • Ability to pass background check and drug screening
- • Proficiency in electronic health record (EHR) systems
