- • Provide skilled occupational therapy services to patients in their homes across the Petaluma area, evaluating individual needs to promote safety, independence, and functional ability in daily living.
- • Conduct comprehensive home assessments to identify environmental hazards and recommend modifications for home safety, including fall prevention strategies and adaptive equipment needs.
- • Develop and implement individualized treatment plans focused on improving mobility, fine motor skills, coordination, and activities of daily living (ADLs) such as dressing, bathing, and eating.
- • Educate patients and family caregivers on proper techniques for medication management, chronic disease control, dementia care, and safe transfer methods to reduce risk of injury.
- • Assess and recommend appropriate wheelchair seating, positioning systems, and orthotic devices to enhance comfort, posture, and functional independence.
- • Collaborate closely with Case Managers and the interdisciplinary care team to align therapy goals with overall patient care plans and medical recommendations.
- • Serve as a patient advocate by communicating clinical observations, progress, and concerns to physicians, nurses, and other healthcare providers involved in the patient’s care.
- • Document all patient interactions, assessments, interventions, and outcomes accurately and in compliance with regulatory standards and agency protocols.
- • Participate in ongoing professional development and training opportunities provided by 21HHS to maintain licensure and enhance clinical expertise.
- • Maintain consistent field presence by traveling to patient homes daily, ensuring timely visits and continuity of care across assigned caseloads.
- • Adhere to all agency policies regarding infection control, patient confidentiality, and safety procedures while working in diverse home environments.
- • Contribute to the agency’s mission of delivering compassionate, family-centered care by building trust with patients and their loved ones through empathy and clear communication.
- • Support the agency’s high standards of care by helping maintain hospital readmission rates below 10%, significantly outperforming the industry average.
- • Engage in team-based problem solving and peer support initiatives fostered by 21HHS to promote clinician well-being and job satisfaction.
- • Participate in quality improvement activities and patient feedback reviews to continuously enhance service delivery and patient outcomes.
- • Represent 21HHS professionally in the community by upholding the agency’s reputation as a Top Workplace and leader in home health care.
🎯 Requirements
- • Valid California Occupational Therapist license
- • Current CPR certification
- • Valid driver’s license and reliable transportation for in-home patient visits
- • Minimum of one year of experience in home health or skilled nursing settings
- • Ability to work independently in diverse home environments while adhering to clinical protocols
- • Strong documentation skills in compliance with Medicare and regulatory standards
