- • Provide skilled occupational therapy services to patients in their homes across El Dorado, Placer, Sacramento, and Fairfield territories, evaluating individual needs to promote safe and independent living.
- • Conduct comprehensive home assessments to identify environmental hazards and recommend modifications for safety, mobility, and accessibility, including wheelchair seating, positioning, and adaptive equipment.
- • Develop and implement individualized treatment plans focused on improving activities of daily living (ADLs), fine motor skills, cognitive function, and overall functional independence.
- • Educate patients and family caregivers on medication management, chronic disease management, falls prevention strategies, and dementia care techniques to support long-term home safety.
- • Collaborate closely with Case Managers and the interdisciplinary care team to coordinate patient care, share clinical observations, and adjust treatment plans based on patient progress and changing needs.
- • Serve as a patient advocate by ensuring care plans align with patient goals, cultural preferences, and clinical best practices while maintaining compliance with Medicare, Medicaid, and private payer regulations.
- • Document all patient interactions, evaluations, and interventions accurately and timely in electronic health records, adhering to agency protocols and regulatory standards.
- • Participate in ongoing clinical supervision, team meetings, and professional development opportunities to enhance clinical skills and stay current with home health best practices.
- • Maintain a consistent caseload while traveling to patient residences within the assigned territory, ensuring timely visits and adherence to scheduled treatment plans.
- • Demonstrate empathy, compassion, and professionalism in all patient interactions, reflecting 21HHS’s commitment to treating every patient as if they were family.
- • Contribute to the agency’s low hospital readmission rates by delivering high-quality, evidence-based interventions that reduce risk factors and promote patient stability at home.

🎯 Requirements
- • Current, active license as an Occupational Therapist (OT) in the state of California
- • Valid driver’s license and reliable transportation for daily travel to patient homes
- • Minimum of one year of experience in home health or skilled nursing settings
- • Ability to pass background check, drug screening, and immunization requirements
- • Proficiency in electronic health record (EHR) systems and documentation standards
- • Strong interpersonal skills and ability to educate patients and caregivers effectively
