📋 Description
- • We are seeking a dedicated and clinically grounded LPN or LVN to serve in a dual-function role at our WI facility.
- • The role will serve as the on-site point of contact for our Medicare 855B site inspection process and support our Chronic Care Management (CCM) program.
- • The position is currently 5 days per week, on-site at 444 Reid Street, De Pere, WI 54115.
- • As our clinical program scales and staffing grows, a hybrid schedule (rotating in-office and remote days) will become available.
- • Physical presence during the 10:00 AM to 2:00 PM window is mandatory.
- • The on-site responsibilities include serving as the sole dedicated physical presence at the facility during all posted operating hours, acting as the immediate, professional point of contact when a MAC field inspector arrives unannounced, and maintaining strict physical HIPAA compliance.
- • The clinical responsibilities include facilitating patient enrollment into Flagler's CCM portal, making daily outbound calls and sending text messages to patients to document health status and maintain engagement, and collecting and documenting patient health information.
- • The requirements include an active LPN or LVN license in good standing in the state of WI, minimum 3 years of experience as an LPN or LVN in a clinical office or remote care setting, and proficiency with Zoom, Slack, and online patient enrollment platforms.
- • The preferred clinical background includes experience in orthopedics, physical and rehabilitation medicine, sports medicine, and neurology.
