- • We are seeking a dedicated and clinically grounded LPN or LVN to serve in a dual-function role at our WI facility.
- • The role will involve serving as the on-site point of contact for our Medicare 855B site inspection process and supporting our Chronic Care Management (CCM) program.
- • The position is currently 5 days per week, on-site at 444 Reid Street, De Pere, WI 54115, with a hybrid schedule (rotating in-office and remote days) to be introduced as the clinical program scales and staffing grows.
- • Physical presence during the 10:00 AM to 2:00 PM window is mandatory.
- • The role will involve facilitating patient enrollment into Flagler's CCM portal, making daily outbound calls and sending text messages to patients, collecting and documenting patient health information, and sending evidence-based health support resources to patients.
- • The role will also involve participating in ongoing company training sessions and team check-ins, and maintaining strict patient confidentiality in full compliance with HIPAA regulations and Flagler company policies.
🎯 Requirements
- • Active LPN or LVN license in good standing in the state of WI.
- • Minimum 3 years of experience as an LPN or LVN in a clinical office or remote care setting.
- • Background in healthcare administration, clinic operations, or front-desk management is a plus.
- • Proficiency with Zoom, Slack, and online patient enrollment platforms.
- • Personal laptop with a working microphone, camera, and reliable internet connection.
- • Quiet, dedicated workspace available for remote work when the hybrid schedule is introduced.
