📋 Description

• • Pediatric Registered Nurse role at Imagine Pediatrics PC, a tech-enabled pediatrician-led medical group reimagining care for children with special health care needs through 24/7 virtual-first and in-home medical, behavioral, and social care.
• • Day-to-day responsibilities include providing proactive care and triage for clinical issues, embedding family-centered care philosophy, demonstrating cultural competence, coordinating transitions of care for ED/IP/UC discharges, performing comprehensive assessments of clinical, psychosocial, discharge planning, and financial needs, establishing rapport with patients and families, collaborating with physicians and interdisciplinary teams to establish care plans, communicating care plans to patients/families and healthcare teams, evaluating plan effectiveness, documenting all care management activities, and referring to social work or behavioral health for complex psychosocial issues.
• • The team is interdisciplinary, including MDs, APPs, social workers, navigators, pharmacists, and dietitians, working together to enhance existing care teams with compassion, creativity, and commitment to children with medical complexity.
• • In this role, you will develop expertise in pediatric care coordination, virtual health delivery, family-centered communication, interdisciplinary collaboration, and value-based care models while contributing to a mission-driven startup culture focused on improving access to quality care for medically complex children.
