- • Physical Therapist- Home Care role at Carilion Clinic involves confirming and interpreting physician referrals, performing patient assessments, and planning and implementing individualized treatment programs based on patient age, developmental status, and medical diagnoses in accordance with physical therapy regulations.
- • Day-to-day responsibilities include providing patient care based on physician referrals and age-appropriate assessments, coordinating care through communication with patients, families, therapists, physicians, and healthcare team members, documenting in medical records, supervising PTAs, collaborating with therapy and support staff, and participating in in-services, performance improvement, program development, orientation, committee work, and staff meetings.

🎯 Requirements
- • Graduate of an approved school of Physical Therapy with either a Bachelor’s degree and 5 years of experience, or a Master’s or Doctorate degree
- • Licensed to practice as a Physical Therapist in the Commonwealth of Virginia by the Virginia Board of Physical Therapy
- • AHA BLS certification from a Carilion-approved program within six months of employment
- • One year of clinical experience required for Home Health positions
- • Effective interpersonal, communication, customer service, and teamwork skills
