- • The Population Health Navigator II role at Cleveland Clinic Health System is essential for coordinating patient-centered care and improving health outcomes through proactive population health initiatives.
- • Day-to-day responsibilities include collaborating with care teams to assist in patient navigation for low, moderate, and rising risk patients; performing data gap closure in payor portals; scheduling preventive care visits; assisting with health insurance navigation; facilitating connections to Cleveland Clinic services; reviewing clinical documentation; and supporting care coordination across Ambulatory Care Management, Value Based Operations, and Community Care initiatives.
- • The role operates within the Administration-Finance department and supports the Ambulatory Care Management Department, Cleveland Clinic Community Care, and the Quality Alliance to enhance patient experience, improve quality and safety outcomes, and reduce care costs.
- • In this position, the individual will develop expertise in population health management, care coordination, health insurance navigation, EPIC systems (preferred), and healthcare operations while contributing to meaningful improvements in patient outcomes and healthcare delivery.
🎯 Requirements
- • High School Diploma or GED and three years of recent experience working with patients in a clinic setting, or an Associate’s or Bachelor’s degree and two years of experience
- • Knowledge of data collection, analysis, and proficiency in Microsoft Office
- • Experience providing exceptional customer service and conflict resolution
- • Working knowledge of HIPAA regulations
- • Ability to sit for prolonged periods, possess good visual acuity, and have dexterity to operate a PC and office equipment
- • Must reside in Florida and be available for occasional onsite meetings
