• • The Population Health Nurse Coach role at Health Advocate focuses on driving sustainable behavior change and improving health outcomes for members with complex chronic and behavioral health conditions through compassionate, evidence-based coaching.
• • Day to day, the nurse coach will conduct telephonic and virtual coaching sessions using motivational interviewing, perform comprehensive assessments, develop personalized care plans, coordinate care across providers, monitor member progress, and document outcomes to support adherence and engagement.
