- • The Principal Care Management (PCM) Care Coordinator plays a vital role in supporting behavioral health care delivery for patients in skilled nursing, assisted living, and independent living facilities by performing remote monitoring and care management under medical team supervision.
- • Day-to-day responsibilities include assisting clinicians with gathering patient information between visits using electronic health record systems, reporting monthly on medical concerns such as falls, medication issues, behavioral changes, and hospitalizations, completing PCM assignments for approximately 15 patients per day, escalating issues to the ancillary program manager or Chief Clinical Officer when needed, and supporting team training through shadowing newer employees.
