📋 Description
- • This Registered Nurse Home Health position at SLUHN Health System is vital to delivering high-quality, patient-centered nursing care in home and community settings, directly supporting the organization’s mission of excellence in care, education, and access for all patients regardless of ability to pay.
- • Day to day, the nurse will conduct comprehensive patient assessments, develop and update care plans, coordinate multidisciplinary services, educate patients and families on health maintenance and disease management, maintain accurate clinical documentation, and ensure compliance with agency policies and third-party payer requirements.
- • The role operates within St. Luke’s University Health Network, a mission-driven healthcare system committed to clinical excellence, community outreach, and professional development, where nurses are valued as essential contributors to patient outcomes and organizational success.
