  • • Responsible for the daily management of an assigned patient caseload in a home care hospice setting, ensuring timely care and documentation aligned with the established plan of care and physician orders.
  • • Collects pertinent health data through in-home patient assessments, telephonic contacts, and electronic medical record (EMR) information to evaluate patient status and needs.
  • • Analyzes collected data to identify clinical issues, available resources, and potential risks such as re-hospitalization, falls, or high resource utilization, and initiates interventions to mitigate risks and ensure high-quality outcomes.
  • • Prioritizes patient care situations based on criticality to patient health and welfare, staff safety, and the timeliness of service delivery.
  • • Identifies barriers to care resolution—including language, literacy, or psychological challenges—and escalates or coordinates with appropriate resources or leadership for timely resolution.
  • • Develops, implements, and oversees a comprehensive plan of care that is clearly communicated to patients, families, and the multidisciplinary clinical team to support patient-centered outcomes and satisfaction.
  • • Directs and coordinates the care provided by LPNs and home health aides (HHAs) within the assigned caseload, ensuring adherence to clinical standards and safety protocols.
  • • Performs ordered treatments, therapies, and procedures in full compliance with the State Nurse Practice Act, Home Health Conditions of Participation, JCAHO standards, and agency policies.
  • • Evaluates patient progress toward established goals and adjusts the plan of care as needed to reflect changing clinical conditions and patient needs.
  • • Must be physically capable of walking, standing, sitting, lifting, reaching, stooping, bending, pushing, and pulling; able to lift and support up to 35 pounds when handling patients, equipment, or supplies.
  • • Must possess intact senses of sight, hearing, smell, and touch; demonstrate finger dexterity; maintain critical thinking and concentration abilities; and respond quickly to sudden changes in patient condition.
  • • Requires successful completion of Physical Abilities Testing as a condition of employment.
  • • Communicates effectively in clear, understandable English with patients, families, and team members.

🎯 Requirements

  • • Graduate from an accredited School of Nursing
  • • Current RN license or temporary license as a Registered Nurse Petitioner in the state of work or residence
  • • BLS certification required per policy guidelines
  • • Must hold licensure in a State Nurse Practice Act-compliant jurisdiction or meet National License Compact (NLC) requirements if declaring NLC state as primary residency
  • • BSN preferred
  • • Physical Abilities Testing must be completed
