- • As a Registered Nurse Clinical Care Specialist at 9amHealth, you will be the frontline of our patient engagement strategy. You will leverage your clinical expertise and empathetic communication skills to connect with patients through various digital channels, including chat, phone, and video consultations. Your primary responsibility will be to deliver comprehensive health coaching, focusing on empowering patients to actively manage their chronic conditions and improve their overall well-being. This role demands a proactive and adaptable individual who thrives in a dynamic, remote work environment.
- • Your day-to-day responsibilities will be diverse and impactful. You will conduct thorough symptom triaging, meticulously assessing patient-reported symptoms and potential side effects, and escalating concerns to our dedicated Providers when necessary, ensuring timely and appropriate medical intervention. A significant part of your role will involve providing in-depth education on key cardiometabolic conditions, including diabetes, hypertension, hyperlipidemia, and weight management strategies. You will translate complex medical information into understandable guidance, helping patients grasp their conditions and the importance of adherence to their care plans.
- • Seamless care coordination is paramount. You will act as a central point of contact, facilitating effective communication and collaboration between patients, our clinical Providers, support teams, and other cross-functional members of our clinical team. This ensures a cohesive and integrated care experience for every member. You will also be responsible for creating detailed consult notes and summaries that align with 9amHealth’s established internal protocols, providing essential information for our Providers to make informed decisions. Accurate and consistent documentation of all patient interactions and care provided is critical to maintaining our high standards of quality and compliance.

🎯 Requirements
- • Bachelor of Science in Nursing (BSN) required.
- • Active Registered Nurse license in good standing, with the ability to obtain additional licensure as needed.
- • Minimum of 3 years of direct patient care experience, preferably in an ambulatory or telehealth setting.
- • Strong proficiency with digital tools and telehealth platforms for care delivery.
- • Certified Diabetes Care and Education Specialist (CDCES) certification, or a willingness to pursue certification.
- • Adaptable, self-directed, and comfortable working independently in a remote environment.
