- ⢠Day-to-day responsibilities include conducting comprehensive patient assessments in home settings using Homecare Homebase EMR, evaluating both patient condition and home environment for safety and suitability of care.
- ⢠The RN develops and updates individualized care plans, performs skilled documentation and visit notes, and communicates findings and changes in status to physicians, referral sources, caregivers, and facility management to ensure seamless coordination of care.
- ⢠Clinical duties involve administering medications and treatments as prescribed, including infusion therapy and advanced wound care, while providing direct patient education on disease management, self-care, and treatment adherence.
- ⢠The RN provides direct supervision to nursing support staff such as LVNs and HHAs, ensuring care is delivered according to plan and regulatory standards, and participates in regular case conferences and in-services to maintain clinical excellence and team alignment.
- ⢠Accurate, timely documentation in Homecare Homebase is required for all visits, supporting compliance, billing, and continuity of care, with expectations to meet or exceed 30 productivity points per week.
- ⢠The clinician acts as a patient advocate, facilitating access to necessary resources and services, and maintaining ongoing communication with all stakeholders involved in the patientâs care journey.

- ⢠Current Texas Registered Nurse (RN) license
- ⢠Three years of clinical nursing experience
- ⢠Experience with wound care and infusion therapy
- ⢠Current Texas driverâs license
- ⢠CPR and BLS certification required
- ⢠Proficiency with Homecare Homebase EMR (preferred)
