📋 Description
- • Serve as a Remote Case Manager – Transitions of Care for Alignment Healthcare, providing nursing care coordination to seniors and chronically ill patients transitioning from hospital or skilled nursing facilities to home.
- • Develop individualized discharge plans in collaboration with patients, families, and interdisciplinary care teams to ensure safe and effective transitions.
- • Coordinate care across providers, rehabilitation facilities, home health agencies, and community-based services to support continuity of care.
- • Monitor patient progress post-discharge and adjust care plans based on clinical outcomes, patient feedback, and evolving needs.
- • Educate patients and their families on self-care practices, medication management, warning signs, and follow-up appointments to promote independence and reduce readmissions.
- • Advocate for patient needs by identifying and connecting individuals to essential community resources, including transportation, meal services, and social support programs.
- • Participate in care conferences, quality improvement initiatives, and interdisciplinary team meetings to enhance care delivery and patient outcomes.
- • Ensure compliance with Medicare Managed Care Plan guidelines, insurance regulations, and state-specific healthcare policies in all patient interactions and documentation.
- • Utilize strong clinical judgment to assess patient risk factors, prioritize interventions, and prevent complications during care transitions.
- • Maintain accurate, timely, and compliant electronic health records and case documentation in alignment with organizational and regulatory standards.
- • Collaborate with case management, utilization review, and clinical operations teams to streamline workflows and improve care coordination efficiency.
- • Support organizational mission by delivering compassionate, patient-centered care that prioritizes the dignity and well-being of vulnerable senior populations.
- • Engage in continuous learning and professional development opportunities to stay current with best practices in transitional care and chronic disease management.
- • Operate remotely with flexible scheduling while maintaining high standards of clinical accountability and communication across virtual platforms.
- • Contribute to team-based initiatives aimed at reducing hospital readmissions and improving overall health outcomes for Medicare Advantage enrollees.
🎯 Requirements
- • Active, unrestricted RN license in California
- • 2–3 years of clinical care management experience (3–5 years preferred)
- • Knowledge of Medicare Managed Care Plans, insurance regulations, and community resources
- • Strong communication, problem-solving, and organizational skills
