- ⢠The Remote On-Call Registered Nurse (RN) plays a critical role in ensuring member safety and clinical continuity during after-hours by providing real-time clinical assessment, triage, and care coordination remotely. As the primary clinical contact during evenings, weekends, and holidays, the RN evaluates symptoms, determines acuity, and guides callers on appropriate next stepsâincluding home care instructions or emergency interventionâwhile maintaining accurate documentation in the electronic health record and facilitating seamless transitions between on-call and daytime teams.
- ⢠Day-to-day responsibilities include providing remote triage via phone or virtual platforms using evidence-based clinical protocols; assessing membersâ symptoms to determine the appropriate level of care (home care, urgent escalation, or emergency response); documenting all encounters accurately and promptly in the EHR; communicating with on-call providers, interdisciplinary teams, or emergency services as needed; offering support and education to members and families to promote safety and prevent avoidable ED visits; managing after-hour issues related to member needs, care coordination, and service interruptions; maintaining awareness of organizational policies and escalation workflows for urgent situations; participating in handoff communication to daytime clinical teams to ensure continuity of care; performing call coverage as needed; and carrying out additional duties to support 24/7 operations.

đŻ Requirements
- ⢠Current, unrestricted Registered Nurse licensure in the state of New York
- ⢠Associateâs degree in Nursing (required); Bachelorâs degree preferred
- ⢠Two (2) years of experience working in an acute care setting (required)
- ⢠Two (2) years of experience working in home care and MLTC/MAP (required)
- ⢠Ability to define problems, collect data, establish facts, and draw conclusions
- ⢠Knowledge of local, state, and federal healthcare laws and regulations, including Medicare and Medicaid
