- • The RN Case Manager plays a critical role in delivering Cigna’s whole-person health strategy by serving as a trusted advocate and clinical partner for customers, families, and caregivers, focusing on care coordination, customer engagement, and health navigation to improve outcomes and deliver exceptional customer experience.
- • Day to day, the role involves partnering with customers to assess needs, develop individualized care plans, and document interventions and outcomes in Cigna medical management systems; collaborating with customers and providers to establish goals and evidence-based plans of care; applying motivational interviewing and behavior change strategies to drive engagement; empowering customers to navigate the healthcare system; coordinating care across customers, caregivers, providers, and internal partners via phone and digital channels; collaborating with interdisciplinary teams including pharmacists, nutritionists, behavioral clinicians, and Medical Directors; tracking, analyzing, and documenting daily activity and outcomes to support performance and quality goals; following established policies and procedures; efficiently navigating multiple systems in a fast-paced environment; and completing required training to maintain clinical knowledge, licensure, and certifications.
