- • The RPM Care Coordinator role at Ennoble Care is vital to delivering high-quality, patient-centered care through remote monitoring and care coordination, directly supporting the company’s mission to provide compassionate, in-home healthcare services to patients with chronic conditions and limited mobility across multiple states.
- • Day-to-day responsibilities include creating and managing individualized patient care plans via the electronic medical record system, conducting frequent patient outreach to ensure care compliance and positive outcomes, handling high volumes of inbound and outbound calls with professionalism, documenting all communications accurately, and coordinating with primary care clinicians and team supervisors to address emergent needs and care plan updates.
