- • Lead the longitudinal management of a complex caseload of patients with nuanced and long-term mental health conditions, ensuring their treatment plans remain safe, effective, and aligned with clinical best practices.
- • Conduct specialist follow-up care for patients already under NHS Mental Health teams or those requiring such oversight, with a focus on escalating or intricate clinical needs.
- • Serve as the primary point of contact for external GPs and NHS Mental Health Services to facilitate coordinated, safe, and compliant patient care through robust information sharing.
- • Perform deep-dive clinical reviews for cases requiring higher-level scrutiny beyond routine follow-up, including those with complex medical histories or atypical presentations.
- • Act as the primary intervention point for clinical escalation, providing expert advice to patients and clinical staff when cases move beyond standard care pathways.
- • Function as the organisation’s Safeguarding Lead, overseeing the development, implementation, and audit of safeguarding policies and procedures across all clinical operations.
- • Deliver mandatory safeguarding training to the clinical team and provide specialist supervision to ensure compliance with professional and regulatory standards.
- • Lead complex safeguarding investigations and conduct regular audits of clinical governance frameworks to maintain the highest safety benchmarks.
- • Serve as the subject matter expert for the Multi-Disciplinary Team (MDT) on complex mental health cases and the safe, evidence-based application of cannabis-based medicines (CBPMs).
- • Advise clinicians across the organisation on complex mental health queries, best practices, and risk management strategies for dual diagnosis and long-term conditions.
- • Collaborate with the Governance Team to review clinical incidents, refine screening protocols, and align clinical workflows with professional, legal, and regulatory requirements.
- • Contribute to the development and refinement of mental health risk assessment tools and clinical governance protocols based on frontline insights and emerging evidence.
- • Maintain accurate electronic patient records and conduct video consultations using digital healthcare platforms with full compliance and data security.
- • Travel to London or Reading at least one day per week (or more as needed) to support in-person clinical, governance, and team collaboration activities.
- • Ensure all clinical activities are conducted with a compassionate, patient-centred approach, prioritising safety, dignity, and continuity of care.
🎯 Requirements
- • Valid and active registration with NMC, HCPC, GMC, or GPhC
- • Minimum of 5 years’ clinical experience focused on mental health care, complex risk assessment, and longitudinal case management
- • Proven experience in clinical auditing, risk management, or serving as a safeguarding lead within a healthcare setting
- • Robust understanding of mental health medications, dual diagnosis, and long-term management of complex mental health conditions
- • Confidence in using digital platforms for video consultations and maintaining electronic patient records
- • Exceptional organisational skills and a compassionate, patient-centred approach to managing complex clinical narratives
