- • Sales professional responsible for building and expanding referral relationships to support patient care growth at a nurse-driven home health organization
- • Acts as a key liaison between Adaptive Home Health and healthcare providers to ensure seamless transitions from hospital to home settings and maintain the company’s reputation for quality care
- • Day-to-day responsibilities include visiting patients prior to discharge to set expectations, establishing trust with patients and care teams, and maintaining detailed records of marketing activities and strategic initiatives
- • Proactively identifies and resolves service issues with a focus on patient and referral-source satisfaction, ensuring high standards of care and communication
- • Works independently to grow relationships with hospital case managers, medical assistants, doctors, and other referral sources in a mixed remote/in-person environment
- • Collaborates with clinical staff to uphold compassionate patient care and satisfaction as top priorities in all outreach and engagement efforts
- • Contributes to process improvement by presenting ideas and suggestions to management based on field experience and market feedback
- • Adheres strictly to HIPAA regulations, state and federal laws, accreditation standards, and company policies to protect patient confidentiality and ensure compliance

🎯 Requirements
- • Minimum 2 years of healthcare marketing experience, with preference for candidates with home health industry background

- • Valid driver’s license required for travel to patient visits, referral source meetings, and community outreach activities
