- • Sales professional responsible for driving growth through strategic relationship management with healthcare referral sources, directly contributing to patient acquisition and organizational expansion in the home health sector.
- • Acts as the primary liaison between Adaptive Home Health and key community stakeholders including hospital case managers, physicians, medical assistants, and discharge planners to ensure seamless patient transitions and sustained referral pipeline.
- • Day-to-day responsibilities include visiting patients prior to hospital discharge to set realistic expectations, building trust with patients and their care teams, and maintaining detailed records of all marketing interactions and outreach efforts.
- • Proactively identifies and resolves service issues by prioritizing patient and referral-source satisfaction, ensuring compliance with clinical standards and fostering long-term partnerships.
- • Works independently in a mixed remote/in-person environment to manage territory, schedule visits, and follow up on leads while adhering to all HIPAA, state, federal, and accreditation regulations.
- • Collaborates with clinical teams to align marketing efforts with patient care quality, reinforcing the organization’s nurse-driven reputation for reliability and compassion.
- • Develops and executes strategic initiatives to expand referral networks, tracks performance metrics, and presents process improvement ideas to management based on field insights.
- • Represents the company at community events, healthcare facilities, and provider offices to increase brand visibility and generate new referral opportunities.
- • Maintains up-to-date knowledge of home health regulations, payer requirements, and referral source preferences to tailor outreach effectively.
- • Supports organizational growth by consistently meeting or exceeding referral targets through persistent, ethical, and relationship-focused outreach.
- • Contributes to a culture of accountability and initiative by self-managing workload, documenting activities thoroughly, and seeking feedback for continuous improvement.
🎯 Requirements
- • Minimum 2 years of healthcare marketing experience, with preference for candidates possessing home health industry background.
- • High school diploma or GED equivalent; additional training will be provided on company-specific processes and compliance standards.
- • Valid driver’s license required for travel to patient homes, healthcare facilities, and community outreach sites as part of screening and selection services (SASS).
