📋 Description

• • Serve as a subject matter expert on clinical programs eligible for members, including Health Assessments, protocols, benefits, and services to resolve inquiries and ensure access to care.
• • Conduct outbound and inbound phone calls to members within departmental timeframes, respecting communication preferences such as time of day, channel, and language, and utilizing interpreter services when necessary.
• • Navigate members through available Clinical/Patient Programs by determining eligibility, connecting them to appropriate providers or vendors, and addressing care gaps and campaign targets.
• • Collaborate with internal departments including Member Services and Clinical Teams to facilitate a seamless and supportive member experience.
• • Accurately document member demographics and interactions in real-time across all required systems, ensuring data integrity and timely wrap-up for outcomes reporting.
• • Meet or exceed individual and team performance goals by submitting activity reports in required formats and frequencies.
• • Deliver exceptional customer service by going above and beyond to ensure member satisfaction and support throughout their healthcare journey.
• • Maintain strong computer proficiency and type at a minimum of 40 words per minute to efficiently manage scheduling and documentation tasks.
• • Apply common sense reasoning to follow detailed written and oral instructions, solve problems in standardized situations, and manage multiple tasks in a fast-paced environment.
• • Perform essential physical functions including regular talking, hearing, sitting, using hands to operate computers and phones, and occasional lifting of up to 10 pounds.
• • Work full-time with availability for overtime during Annual Enrollment Period (October–December) and Open Enrollment Period (January–March).
• • Uphold accuracy and attention to detail in all member interactions and data entry, recognizing the critical impact on organizational data quality and member outcomes.
• • Communicate positively and professionally with members, management, employees, and vendors, fostering constructive relationships across diverse populations.
• • Interpret and apply documents such as safety rules, operating instructions, and procedure manuals to support daily responsibilities.
• • Perform basic mathematical operations involving two-digit numbers, currency, weight, volume, and distance as needed for scheduling and reporting tasks.
