- • Serve as a Senior Infusion Nurse providing expert, direct patient care in home and alternate infusion suite settings under a physician’s plan of care with minimal supervision.
- • Initiate, develop, and implement advanced and complex nursing treatment plans, evaluating patient progress toward goals and modifying plans based on patient status or physician orders.
- • Conduct comprehensive patient assessments during each skilled visit using health data and patient interviews, re-evaluating needs through physical reassessment and response to therapy.
- • Obtain and clarify physician orders for treatment revisions, promptly inform physicians of significant patient condition changes, and submit written summaries within required timelines.
- • Deliver patient- and family-centered education to promote independence in prescribed therapy and care, ensuring active participation aligned with treatment goals.
- • Provide training, mentorship, and precepting to lower-level nurses, including orientation and onboarding support.
- • Participate in facility process improvement initiatives and assist with chart audits to ensure compliance and quality of care.
- • Research and recommend changes to care delivery practices to align with nursing best practices and regulatory standards.
- • Communicate effectively and in real-time with clinical teams, medical providers, patients, and families to ensure continuity of care and timely documentation.
- • Complete all clinical documentation legibly and accurately during visits, applying approved abbreviations and error correction practices per Option Care policies to support billing optimization.
- • Coordinate discharge planning, prepare detailed discharge summaries with patient instructions, and report care needs, progress, and goals when transferring patient care.
- • Ensure full compliance with agency operations, Option Care policies, professional standards, and local, state, and federal regulations including HIPAA and accreditation requirements.
- • Utilize computers and automation technologies to validate treatment orders, document care provided, and adhere to assigned visit schedules.
- • Respond promptly to patient requests and initiate emergency procedures as necessary.
- • Maintain ethical and professional conduct, accepting accountability for clinical practice and adherence to Option Care’s mission, values, and objectives.
- • Safeguard patient and proprietary company information in compliance with legal and ethical confidentiality standards.
- • Educate patients on financial obligations related to care and service charges, speaking knowledgeably about Option Care’s scope of services.
- • Complete required training modules, in-services, and continuing education to maintain licensure and competency in home and alternate care settings.
- • Provide oversight and coordination of paraprofessionals in the home setting and may act as supervisor in the absence of the Nurse Manager or Supervisor as permitted by regulation.
- • Participate in nursing department on-call responsibilities.
- • Insert PICC/Midlines per physician order, troubleshoot and declot obstructed central line catheters, and monitor lines while educating patients on infection prevention and routine care.
- • Perform duties requiring lifting up to 50 pounds (maximum 75 pounds), prolonged standing, walking, bending, reaching, and fine motor skills with visual acuity.
- • Travel up to 100% of the time for business purposes to provide care in patients’ homes and alternate sites across the Greater Philadelphia region.
- • Adhere to safety protocols including use of closed-toe shoes, PPE (gloves, gowns, masks, goggles), and functional communication devices in routine and emergency situations.
- • Work in environments with potential exposure to blood, body fluids, infectious materials, hazardous substances, sharps, chemicals, extreme weather, and unpredictable home settings.
🎯 Requirements
- • Active and unrestricted RN license in the state of Pennsylvania
- • CRNI certification required
- • BSN with at least 5 years of nursing experience OR at least 7 years of nursing experience with 3 years of infusion experience
- • Current CPR certification
- • Valid driver’s license and access to a personal vehicle with proof of liability insurance
- • Demonstrated competency in infusion nursing care and patient safety standards
