- • **Lead and Execute a Vital Patient Support Program:** As a Senior Nurse Navigator for the Thera Nurse Navigator Program, you will be at the forefront of delivering exceptional patient care and program leadership. This remote role is crucial for ensuring the highest standards of patient education, engagement, and ongoing support across the United States. You will not only provide direct, hands-on clinical navigation but also spearhead the overall execution and success of the program, acting as a vital link between patients, healthcare providers, and the program's strategic objectives.
- • **Drive Program Excellence and Strategic Alignment:** You will serve as a key strategic partner to the Executive Director of Nursing Program Deployment. Your responsibilities will include driving operational excellence, ensuring consistent and compliant patient-centered services, and contributing to the strategic vision of the program. This involves a deep understanding of program goals, adherence to stringent compliance standards, and meeting and exceeding client expectations.
- • **Empower and Develop a High-Performing Nursing Team:** This role involves significant leadership responsibilities, including direct oversight, coaching, and quality assurance for a team of Nurse Navigators. You will be instrumental in their professional growth, conducting performance evaluations, providing ongoing mentorship, and fostering a culture of continuous learning and improvement. Your ability to act as a 'player coach' means you'll balance your own clinical duties with the development and support of your team.
- • **Deliver Expert Patient Education and Engagement:** Provide high-quality, evidence-based education to patients regarding their therapy, disease state, treatment expectations, and adherence strategies. You will be a subject matter expert in patient engagement best practices, continuously working to elevate the team's capabilities in patient-centric communication. This includes monitoring key engagement metrics and proactively implementing strategies to enhance outreach, improve the quality of education, and boost patient satisfaction.
- • **Facilitate Telephonic and Virtual Patient Interactions:** Engage directly with patients and healthcare providers through telephonic and virtual channels. - • **Lead Team Development and Performance Management:** Conduct thorough annual performance reviews for the Nurse Navigator team, assessing clinical competencies, productivity, and identifying individual professional development needs. Lead regular team huddles, disseminate training updates, and champion professional development initiatives to keep the team at the cutting edge of patient navigation.
- • **Manage Escalations and Resolve Complex Issues:** Act as a primary point of escalation for patient or provider inquiries that require a higher degree of clinical expertise or sensitive handling. Your role will be to expertly resolve complex customer issues and complaints, ensuring patient satisfaction and program integrity.
- • **Drive Reporting and Data-Informed Improvements:** Prepare and present comprehensive monthly team activity reports. These reports will encompass critical data such as patient engagement metrics, operational performance, quality outcomes, and identified opportunities for improvement. Utilize these data insights to inform strategic decision-making and drive continuous program enhancement.
- • **Proactive Risk Identification and Mitigation:** Ensure the timely escalation of any identified risks, challenges, or compliance concerns to leadership. Crucially, you will also propose potential solutions, demonstrating a proactive and problem-solving approach to program management.
