- • As a Primary Care at Home Social Worker II, you will play a vital role in supporting patients with chronic and complex medical needs in the comfort of their own homes.
- • You will provide compassionate counseling, care planning, and ongoing support while addressing social determinants of health and helping patients and families navigate challenges.
- • Working closely with an interdisciplinary care team, you will promote patient stability and independence, support care transitions, and connect individuals to community resources and services that enhance quality of life.

🎯 Requirements
- • Master’s degree in social work (MSW) from an accredited graduate school of Social Work
- • Minimum of 1-2 years of related work experience preferred
- • Delaware Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) preferred
