- • Provide skilled speech-language pathology services to patients in their homes across Solano County, including Vallejo, American Canyon, and Hercules, with minimal visits to Napa County.
- • Evaluate and treat speech, language, cognitive, and fluency disorders that impact communication abilities in adult and pediatric patients.
- • Assess and manage swallowing disorders (dysphagia) to ensure safe and effective oral intake and reduce aspiration risk.
- • Collaborate closely with Case Managers and the interdisciplinary care team to develop, implement, and adjust individualized treatment plans aligned with patient goals and medical needs.
- • Educate patients and their families on communication strategies, swallowing techniques, and home-based exercises to promote continuity of care.
- • Advocate for patients by identifying and addressing barriers to communication, nutrition, and functional independence within their home environment.
- • Document patient progress accurately and timely in accordance with clinical, regulatory, and insurance requirements.
- • Participate in care planning meetings and interdisciplinary team conferences to ensure coordinated, patient-centered care.
- • Maintain compliance with all state and federal regulations governing home health services, including Medicare and Medicaid guidelines.
- • Travel to patient residences daily to deliver in-home therapy services; no remote or work-from-home arrangements are available.
- • Contribute to a culture of compassion and excellence by upholding 21HHS’s commitment to treating every patient as if they were family.
- • Engage in ongoing professional development opportunities offered by 21HHS to enhance clinical skills and stay current with evidence-based practices.
- • Support organizational goals by helping maintain hospital readmission rates below 10% through high-quality, consistent patient care.
- • Represent 21HHS professionally in patient homes and within the community, reinforcing the agency’s reputation as a Top Workplace and leader in home health.
🎯 Requirements
- • Valid California license as a Speech-Language Pathologist (SLP)
- • Certificate of Clinical Competence (CCC-SLP) from ASHA
- • Current CPR certification
- • Valid California driver’s license and reliable transportation for in-home patient visits
- • Experience providing speech-language pathology services in a home health or home care setting
- • Ability to work independently in diverse home environments while adhering to clinical standards
