  • • Serve as a Senior Infusion Nurse providing expert, direct patient care in home and alternate infusion suite settings under a physician’s plan of care with minimal supervision.
  • • Initiate, develop, and implement advanced and complex nursing treatment plans, evaluating patient progress toward goals and modifying plans based on patient status or physician orders.
  • • Conduct comprehensive patient assessments during each skilled visit using health data and patient interviews, re-evaluating needs through physical reassessment and response to therapy.
  • • Obtain and clarify physician orders for treatment revisions, promptly inform physicians of significant patient condition changes, and submit written summaries within required timelines.
  • • Deliver patient-centered education to patients and families/caregivers to promote independence with prescribed therapy and care needs, aligning with established treatment goals.
  • • Provide training, mentorship, and precepting to lower-level nurses, including orientation and onboarding support.
  • • Actively participate in facility process improvement initiatives and assist with chart audits as assigned by supervisors or Nurse Managers.
  • • Research and recommend evidence-based changes to care delivery practices to align with nursing best practices and regulatory standards.
  • • Communicate effectively and in real-time with Option Care’s clinical team, medical providers, patients, and families to ensure continuity of care.
  • • Complete all clinical documentation legibly and in real-time during visits, adhering to Option Care’s policies on abbreviations, error correction, and billing compliance timelines.
  • • Coordinate discharge planning, prepare detailed discharge summaries with patient instructions, and report care needs, progress, and goals during care transitions.
  • • Ensure full compliance with Option Care’s policies, professional standards, and all applicable local, state, and federal regulations including HIPAA and accreditation requirements.
  • • Utilize computers and automation technologies to validate treatment orders, document care provided, and adhere to assigned visit schedules for maximum efficiency.
  • • Respond promptly to patient requests and initiate emergency procedures as necessary.
  • • Maintain ethical and professional conduct, accepting accountability for clinical practice and adherence to Option Care’s mission, values, and objectives.
  • • Safeguard patient and proprietary company information in full compliance with HIPAA and legal confidentiality guidelines.
  • • Educate patients on financial obligations related to care and services, speaking knowledgeably about Option Care’s scope of offerings.
  • • Complete all required training modules, in-services, and continuing education to maintain licensure and competency in home and alternate care settings.
  • • Provide oversight and coordination of paraprofessionals in the home setting and may act as supervisor in the absence of a Nurse Manager, per state and federal regulations.
  • • Participate in nursing department on-call responsibilities as assigned.
  • • Insert PICC/Midline catheters per physician order, troubleshoot and declot obstructed central lines, and educate patients on infection prevention and routine line care.
  • • Demonstrate ability to lift up to 50 pounds routinely and up to 75 pounds occasionally, with physical demands including walking, standing, crouching, kneeling, reaching, and fine motor skills.
  • • Operate a personal vehicle for business travel up to 100% of the time, with valid state license and proof of liability insurance.
  • • Adhere to safety protocols including use of closed-toe shoes, PPE (gowns, gloves, masks, goggles), and functional communication devices for routine and emergency situations.
  • • Accept potential occupational exposure to blood, body fluids, infectious materials, hazardous substances, sharps, chemicals, and unpredictable home environments.
