📋 Description

• The PRN After-Hours Triage RN provides remote clinical triage, problem-solving, and urgent patient support outside of normal business hours, ensuring timely clinical decision-making and continuity of care for home health patients requiring after-hours intervention.
• The role involves responding to after-hours patient calls, conducting telephonic assessments, providing clinical guidance for symptoms and condition changes, documenting triage calls in the EMR, determining need for in-person visits, and escalating emergencies to 911 or urgent care when necessary.
• Fira Health, Inc. is a home health agency focused on delivering quality patient care through clinical operations, with this role reporting to the VP of Operations/Clinical and supporting field clinicians, caregivers, and facility partners during evenings, weekends, and holidays.
• The RN will develop advanced telephonic assessment skills, strengthen clinical judgment in urgent scenarios, gain experience in home health regulatory compliance, and enhance independent decision-making and documentation proficiency in a fully remote setting.

🎯 Requirements

• Active RN license
• 1–2 years recent clinical experience (home health, ER, urgent care, med-surg, or ICU preferred)
• Strong triage and telephonic assessment skills
• EMR proficiency (HCHB preferred)
• Reliable internet and ability to work independently
