📋 Description

• Advanced Practice Provider (NP or PA) role in Devoted Health’s virtual Palliative Care program, focused on delivering compassionate, patient-centered care to members with life-limiting illness in their final year of life.
• Day-to-day responsibilities include managing a longitudinal panel of 40-50 high-risk patients via telephonic and video encounters, conducting initial assessments, developing palliative plans of care, providing symptom management and goals of care counseling, supporting care transitions, evaluating hospital admissions, and participating in team huddles, interprofessional meetings, and panel reviews.
• Devoted Health is a mission-driven, fast-growing healthcare company founded in 2017 that combines health insurance, clinical care, service, and technology to improve care for older Americans, with a commitment to treating members like family and advancing healthcare equity.
• The role offers the opportunity to build the foundation of a novel virtual palliative care program, develop clinical and leadership skills in a startup environment, contribute to reducing readmission rates, and make a meaningful impact on vulnerable patient populations through innovation and compassionate care.

🎯 Requirements

• Unrestricted advanced practice license in 1 or more of the Devoted Membership states: FL, TX, AZ, OH, CO, OR, TN, IL, AL, HI, NC, SC, PA
• 5 years in advanced practice, with at least 3 years in Palliative care
• ACHPN® credential, or ability/willingness to obtain within first six months of employment
• Willingness to flex between time zones
• Comfortable with technology and a fast-paced environment
• Self-starter with ability to succeed in a work-from-home environment
