📋 Description

• Serve as the primary point of contact for ALTCS members, explaining program rights, responsibilities, grievance procedures, and appeal systems in compliance with AHCCCS regulations.
• Conduct comprehensive bio-psychosocial assessments of members in accordance with AHCCCS timeframes, documenting functional, physical, and behavioral health status while identifying individual strengths and needs.
• Develop, implement, and regularly update individualized service plans based on member preferences, strengths, and needs, ensuring services are delivered in the most cost-effective and integrated setting.
• Authorize and coordinate care with provider agencies to ensure members receive appropriate services aligned with their assessed needs and placement preferences.
• Act as an advocate and facilitator for members when navigating issues with healthcare providers, community programs, or regulatory agencies.
• Function as a gatekeeper to ensure members receive only the most appropriate and necessary services, avoiding unnecessary or redundant care while maintaining budgetary compliance.
• Conduct in-person assessments at members’ homes and facilities within Pima County, with all other duties performed remotely.
• Collaborate with interdisciplinary healthcare teams including nursing staff, medical providers, and case management personnel to support holistic member care.
• Maintain accurate and timely documentation of all member interactions, assessments, service plans, and authorizations to meet regulatory and organizational standards.
• Support members in defining personal goals, identifying barriers to achievement, and encouraging self-resolution of challenges within their care environment.
• Engage with external stakeholders including physicians, payers, community agencies, and regulatory bodies to coordinate care and ensure compliance.
• Work a standard Monday through Friday schedule from 8:00 AM to 5:00 PM, with occasional travel required within Pima County for home and facility assessments.
• Operate within the framework of Banner Plans & Networks (BPN), an accountable care organization focused on integrated, cost-efficient healthcare delivery for Arizona’s elderly and disabled populations.
• Ensure all activities align with Banner Health’s mission to improve health outcomes through compassionate, coordinated care.

🎯 Requirements

• Bachelor’s degree in Social Work
• Two years of experience serving elderly individuals and/or persons with physical disabilities or Serious Mental Illness (SMI)
• Knowledge of ALTCS program regulations and AHCCCS timeframes for assessments
