- • The Nurse Practitioner or Physician Assistant at Nest Health serves as the primary care provider for underserved families, delivering whole-family primary, acute, chronic, and pediatric care in mobile/community and virtual settings to advance Nest’s mission of making comprehensive, value-based care radically accessible to high-risk populations.
- • Day-to-day responsibilities include managing a panel of families, providing longitudinal preventive and chronic care, utilizing electronic health records to document interactions and coordinate care, attending team huddles and case conferences, prescribing medications, ordering diagnostic tests, offering health and social service navigation, and participating in community outreach to educate families about Nest’s programs.
- • Nest Health is a value-based care startup founded in 2022 by former Louisiana Secretary of Health Dr. Rebekah Gee, delivering integrated medical, behavioral, and social care through in-home and virtual models, with national traction and partnerships expanding access to thousands of high-risk families across multiple states.
- • In this role, you will develop clinical leadership skills in interdisciplinary team settings, gain experience in startup healthcare innovation, deepen expertise in managing chronic and behavioral health conditions across the lifespan, and contribute to reducing health disparities through culturally congruent, patient-centered care.
🎯 Requirements
- • Completion of an accredited Physician Assistant or Nurse Practitioner program with an active, unrestricted license to practice in Arizona
- • At least 2 years of clinical experience providing care to both pediatric (including newborns) and adult patients with chronic medical and behavioral health conditions
- • Demonstrated proficiency in clinical skills such as venipuncture, patient assessments, and care plan reinforcement for conditions like diabetes, hypertension, depression, and CHF
- • Ability to work full-time on a four-day, 10-hours-per-week schedule with rotating call including occasional evenings and weekends
- • Current CPR (BLS) certification and access to reliable transportation with willingness to comply with Nest’s Motor Vehicle Safety Policy
