- • The Behavioral Health Case Manager supports members transitioning from inpatient behavioral health care back into the community, playing a critical role in ensuring continuity of care and successful recovery outcomes.
- • Day-to-day responsibilities include conducting onsite visits to local acute inpatient psychiatric facilities in the Memphis area at least twice weekly, participating in treatment team meetings, helping members identify appropriate behavioral health services and community-based resources post-discharge, and providing telephonic follow-up to support ongoing recovery.
- • The role is part of the Behavioral Health Transition Care Team at BlueCross BlueShield of Tennessee, a mission-driven organization focused on improving health outcomes for members across Tennessee through coordinated, compassionate care.
- • In this position, you will develop expertise in behavioral health case management, discharge planning, and community resource coordination while building strong collaborative relationships with providers, internal teams, and members in a dynamic, high-volume environment.
🎯 Requirements
- • Current, active unrestricted Tennessee license in Nursing (RN) or behavioral health field at Master’s level or above (e.g., LCSW/LMSW, LLP, MHC, LPC, Ph.D.)
- • Minimum 3 years of clinical behavioral health or substance use disorder experience
- • At least 1 year of experience or knowledge regarding community care resources and levels of behavioral health care available
- • Proficiency in Microsoft Office (Outlook, Word, Excel, PowerPoint)
- • Strong interpersonal, organizational, and analytical skills with excellent oral and written communication abilities
- • Experience with Motivational Interviewing Techniques and Adult Learning Styles
