Bilingual Care Coordinator, Transition of Care (Remote, Mon-Fri, 8am-5pm Pacific Required)

📋 Description

• • As a Bilingual Care Coordinator in the Transitions of Care team, you will partner closely with RN case managers to support seniors and chronically ill members through critical transitions following hospital or skilled nursing facility discharge, ensuring timely access to care and services.
• • Your day-to-day responsibilities include creating and managing case documentation, reaching out to members by phone to coordinate home health, durable medical equipment, medication refills, and provider appointments, requesting and uploading medical records, and escalating concerns to nurses when members show signs of non-compliance or changing conditions.
• • You will work within Alignment Health’s mission-driven, fast-growing organization dedicated to transforming senior care, collaborating with a team of passionate professionals focused on putting seniors first and improving health outcomes for vulnerable populations.
• • In this role, you will develop expertise in care coordination, managed care systems, Medi-Cal, and healthcare navigation while building strong member relationships, advocating for patient needs, and contributing to reduced care gaps and improved health outcomes for seniors with complex chronic conditions.
