📋 Description

• Conduct virtual clinical assessments for cardiology patients in both English and Spanish, focusing on general cardiology conditions and heart failure management.
• Deliver 80-90% of care through virtual visits using a proprietary electronic health information platform.
• Formulate accurate diagnoses and develop individualized treatment plans for heart failure and general cardiology patients, including medication management, volume monitoring, and lifestyle modifications.
• Initiate, titrate, and manage guideline-directed medical therapies (GDMT) for heart failure patients.
• Assess and intervene in weight fluctuations and acute volume overload to prevent hospitalizations.
• Order and interpret in-home diagnostics such as echocardiograms, EKGs, and Holter monitors; coordinate community-based diagnostics like stress testing and advanced cardiac imaging.
• Collaborate with an interprofessional team including RNs, physicians, pharmacists, social workers, and medical assistants to ensure holistic, coordinated care.
• Serve as a clinical advisor and provide escalation support for specialty clinic staff and other teams during business hours.
• Maintain accurate, compliant patient medical records in alignment with legal and ethical standards.
• Participate in quality improvement initiatives and ongoing professional development to stay current with cardiovascular best practices.
• Provide feedback to improve the company’s home-grown electronic health information system and telehealth technology.
• Adhere to all telehealth regulations, patient privacy laws, and industry standards.
• Work within Central, Mountain, or Pacific Time Zone during standard business hours (8-5).
• Obtain and maintain active, clear RN and APRN or Physician Assistant license in AL, AZ, GA, IL, OH, and/or TX at time of hire and throughout employment.
• Support the mission of expanding access to high-quality, virtual-first cardiovascular care for vulnerable seniors.

🎯 Requirements

• Active and clear RN and APRN or Physician Assistant license in AL, AZ, GA, IL, OH, and/or TX at time of hire
• Bilingual proficiency in English and Spanish for conducting comprehensive clinical assessments
• 5 or more years of cardiology clinical experience, ideally with 2 years in an outpatient cardiology setting
• Experience conducting telehealth visits via video platforms and comfort with virtual care delivery
• Secure and private workspace with reliable internet connection
• High comfort level with EMRs, messaging platforms, online portals, and AI tools
